Problem
Patients with chronic conditions were forgetting medications, missing follow-ups, and burying clinicians in repetitive calls. The existing app felt like a database, not a companion.
Solution
A calm, ritual-driven app: morning briefings, ambient reminders, and a 'today' surface that hides everything not actionable. Built a clinician-facing escalation layer to keep both sides in sync.
